Cusack tragically passed away on September 20, 2023, at just 27 years old. The cause of her death, however, was not disclosed, leaving fans questioning the circumstances surrounding this tragic event.

From the outset, the media received incomplete information. It's known that Cusack was found deceased at her residence. In response, the police issued a succinct statement declaring that, according to their investigation, her death "is not suspicious".

Still, this did not bring closure. A speech made by the player's mother during the funeral raised concerns. She voiced, "In February of this year, they allowed an untamed and unshackled spirit to be crushed. A spirit named Maddy, which we fervently safeguarded."

Despite these concerns, the family assured that Cusack had no mental health issues. Suspicions arose that circumstances within the club might have had an impact on the player's outcome. Consequently, Sheffield United commenced their investigation. What did they find?

"After meticulously examining the information given by Maddy's family and other witnesses, the investigation did not expose any evidence of misconduct", the English club affirmed.

Maddy Cusack was a former youth representative of England. Although she had previously played for Aston Villa, Birmingham City, and Leicester City, she only managed to leave her mark at Sheffield United where she was consistently given opportunities. For a while, she served not just as a player, but also worked within the club's marketing department.

After her death, a minute of silence was observed during the men's Premier League match between Sheffield United and Newcastle United. Cusack was featured on the cover of the match program, and in the eighth minute, the whole stadium began to applaud in honor of the player who donned the number eight jersey.
